Overview
Many UK transplant units apply a BMI threshold for listing, commonly around 35 kg/m2, because higher body weight increases surgical and wound complications. That threshold is a clinical judgement, not an absolute rule, and it varies between units.
Why it's done and who it helps
People held back from listing by weight alone often have the most to gain from a structured, monitored programme with clear timelines.
What a good programme looks like before listing
- A realistic weight target agreed with your transplant unit, not an arbitrary number.
- Preservation of muscle mass and fitness, since deconditioning also affects surgical risk.
- Careful nutrition planning if you are on dialysis while losing weight.
- Documented, auditable monitoring that your unit can rely on in its assessment.

Will losing weight guarantee listing?
No. Weight is one of several factors your unit assesses, but it is often the modifiable one.
Should treatment stop before surgery?
Yes, timing is planned in advance because of delayed stomach emptying and anaesthetic fasting rules.
When to contact your care team
Contact the clinic promptly if you have any of the following:
- Vomiting or diarrhoea lasting more than 24 hours, or you cannot keep fluids down
- Passing much less urine than usual, or new swelling of the legs or face
- Severe pain in the upper abdomen that spreads through to your back
- A weight change of more than 2 kg in a week that is not explained by fluid
- A missed weekly weight, a missed dose, or any hospital admission
