Protein timing and strength during weight loss

How to combine protein intake and training so that weight lost is fat rather than muscle.

Overview

Muscle is preserved by two things together: a training stimulus and enough protein. Either one alone is much less effective, and on a sharply reduced appetite the combination needs planning.

Why it's done and who it helps

Anyone losing weight quickly, and particularly older adults, dialysis patients and transplant recipients on steroids.

Practical structure

  • Spread protein across the day rather than one large meal, within the target your renal dietitian sets.
  • Have a protein-containing meal or snack within a few hours of training.
  • Train the major muscle groups at least twice weekly.
  • Track a function measure, such as how many sit-to-stands you can do in 30 seconds, alongside your weight.

Signs the balance is wrong

Weight falling faster than roughly 1% a week, new weakness, breathlessness on stairs that is new, or a falling grip strength. Any of these prompts a review of dose, protein and training.

Only within an individualised target. Never start high-dose supplements without dietetic advice.

How long before I notice strength gains?

Most people notice functional improvement within four to six weeks.

When to contact your care team

Contact the clinic promptly if you have any of the following:

  • Vomiting or diarrhoea lasting more than 24 hours, or you cannot keep fluids down
  • Passing much less urine than usual, or new swelling of the legs or face
  • Severe pain in the upper abdomen that spreads through to your back
  • A weight change of more than 2 kg in a week that is not explained by fluid
  • A missed weekly weight, a missed dose, or any hospital admission
