What happens when you stop a GLP-1 medicine

Overview

Stopping a GLP-1 medicine removes the appetite signal within a few weeks. Trial data show most people regain a substantial share of lost weight within a year without a maintenance plan.

Why it's done and who it helps

• Appetite typically returns over three to six weeks.
• Average regain in trials is roughly two-thirds of lost weight within 12 months.
• Muscle mass lost during treatment is not automatically regained.
• A planned taper with a maintenance strategy substantially reduces regain.

Planned versus unplanned stopping

Unplanned stops — supply issues, cost, side effects — produce the worst outcomes. Tell the clinic before you stop so a taper and a plan can be arranged.

Building the maintenance plan

Protein target, two to three resistance sessions weekly, weekly weighing and a defined 'action weight' at which you contact the clinic.

Restarting

Restarting later is usually possible but begins again at a low escalation dose.

When to contact your care team

Contact the clinic promptly if you have any of the following:

• Vomiting or diarrhoea lasting more than 24 hours, or you cannot keep fluids down
• Passing much less urine than usual, or new swelling of the legs or face
• Severe pain in the upper abdomen that spreads to your back, with or without vomiting
• A weight change of more than 2 kg in a week that is not explained by fluid
• A missed weekly weight, a missed dose, or any hospital admission
